What is Frozen Shoulder?

WHAT IS FROZEN SHOULDER 

 

In other terms is known as Adhesive Capsulitis, a condition where an insidious painful condition of the shoulder persists more than 3 months.This inflammatory condition causes fibrosis of the glenohumeral joint capsule is accompanied by a gradually progressive stiffness and significant restriction of range of motion most commonly the external rotation movement of the shoulder. The structure usually affected first is the coracohumeral ligament, the roof of the rotator cuff  interval. Contraction of the coracohumeral ligament limits external rotation of the arm, which is usually first affected in early stages. In advanced stages, thickening and contraction of the glenohumeral joint capsule develop, further limiting the range of motion in all directions.(Mezian, K., et al, 2023). It is also characterized by a painful restricted range of passive and active movement in all planes of motion (Lyne, S.A.,et al, 2022). FS is commonly seen in the population lying in the age group 40-60. The incidence of FS is less common after the 70s except in certain cases with a traumatic shoulder injury.(Phansopkar, P.et, al., 2022).  Females are 4 times more often affected than men, while the non-dominant shoulder is more prone to be affected (Mezian, K,. et, al., 2023). The prevalence among the diabetic population and those suffering from pathologies of the thyroid gland, especially hypothyroidism may increase as high as 10 to 38% (Daniel, D., et al, 2021).

COMMON SYMPTOMS 

  • sudden onset of unilateral anterior shoulder pain (Mezian, K,. et, al., 2023), without any preceding traumatic, infective, or inflammatory event (Pandey, V., et, al., 2021)
  • passive and active range of motion restriction, first affecting external rotation and later abduction of the shoulder
  • limited reaching, particularly during overhead (e.g., hanging clothes) or to-the-side (e.g., fasten one's seat belt) activities 
  • neck pain, mostly derived from overuse of cervical muscles to compensate the loss of shoulder motion
  • Pain is usually poorly localized, around the shoulder and is almost always troublesome in the night while the patient lies on the affected side (Pandey, V., et, al., 2021)
