Fore foot pain (1st, 2nd or 3rd toes), Also known as Metatarsalgia is
very common.
But many patients never get the best treatment.

Because everyone is still treating the pain area.
Besides, fore foot pain does not have the pain starts from fore foot.



Symptoms  :

• Pain and tenderness of plantar surface of the heads of the metatarsal bones or metatarsal joints
• Swelling on top of the metatarsal joint
• Callus formed under the prominent metatarsal heads
• Increased pain during mid-stance and propulsion phases of walking, as body weight is shifted forward onto forefoot
• Feels deep bruise, walking on rocks under ball of foot
• Worsens when walking/standing barefoot on a hard surface, poorly cushioned shoes.

 
Associated condition: 

• Tight toe extensor muscles
• Tight Achilles tendon
• Change or unusual levels or physical activity, landing games
• Hammertoe deformity 
• A high arch 
• Ballet / Gymnast
• Tip Toeing Gait

Special test 
What are the common tests used to identify Metatarsalgia ?

• Foot Posture Index
• Tight Achilles tendon, reduced dorsiflexion
• Overpronated (Flat Feet) / Oversupinated (High Arch) foot
• Tenderness/positive Tinel sign over major nerve trunks 

Self Management

• Footwear modification
• Intrinsic foot exercises (if hypermobile foot / weak tibialis post)

Seated calf stretch, hold for 10s,repeat 3-5times



Fascia Stretch, hold for 15s, repeat for 3-5 times


Standing calf stretch, hold for 15 seconds, 3-5 reps


Toe extensor / downward stretch, hold for 15 seconds, 3-5 reps
