Sacroiliac Joint Dysfunction (SIJD)
SIJD is one of the types of lower back pain. Diagnosing and treating SIJD lies in its inconsistent presentation. However pain usually will trigger on lower back (center of sacrum area), buttock, leg, groin and hip. It works through multiple mechanisms and muscle function. The muscles surrounding the SIJ serve to provide muscular forces, guide movement and enhance stability to the pelvic bone.
Muscles that reinforce and stabilize SIJ:
- Erector Spinae
- Multifidus
- Abdominal Muscles
- Hamstring
Sign and symptoms:
- Unspecific sustained positions
- Stiff in bending
- Weak when climbing and rising
- Frequently urinate
- Numbness and tingling on buttock
- Uncomfortable during intercourse
- Tightness pain on the hamstring, quadriceps and hip flexor
Physical Assessment:
Faber Test – Hip Flexion, Abduction and External Rotation to identify presence of hip, lumbar spine and sacroiliac pathology to reproduce pain in this region.
Hip Thrust Test – Knee and hip flex 90°, hold and pull the affected side of pelvic anteriorly; stabilize the non-affected side of pelvic, pain produces positive finding.
Overhead Squat, to assess knee genu valgus or varus, anterior pelvic tilt, excessive lumbar flexion and asymmetrical weight shift.[1]
Treatment
- Manual therapy - Myofascial Release and trigger point on the imbalance muscle that changes the position of SIJ or compensation movement of other conditions that make the SIJ asymmetry and protrusion.
- Stretching
Lengthening the hip flexor and external rotator muscles which in shortening condition due to dynamic muscle, reducing hyperactivity of gluteus muscles and latissimus dorsi.
- Strengthening – Activation of gluteal muscle weakness, reducing of shearing forces across the SIJ
- Stabilizing pelvic floor muscles – engagement of pelvic floor muscles during do an activity
- Correcting posture & gait abnormalities
- Modalities - To enhance the recovery process on the affected part
